In a groundbreaking shift for cardiovascular health, new research published in late February 2026 suggests that the current standards used to assess heart disease risk may be failing women by underestimating the danger of “minor” arterial plaque. The Findings: A Lower Threshold for Danger

The most startling discovery was the difference in “risk trajectories.” For men, heart risk began to climb when their total plaque burden reached about 28%. For women, that danger zone started much earlier—at just 20%. Furthermore, once that 20% threshold was crossed, the risk for women increased much more steeply and aggressively than for men.
Why the Difference?
Medical experts believe the disparity is rooted in basic biology. Women typically have smaller coronary arteries than men. Because the vessels are narrower, a small amount of plaque occupies a larger percentage of the available space, leading to more significant obstructions earlier.
“Our findings suggest that applying uniform thresholds across sexes… may underestimate risk in women,” noted Dr. Brendel. Essentially, what a doctor might call “mild” plaque in a man could be “moderate” or “high-risk” in a woman due to the scale of her cardiovascular system.
A Call for Personalized Diagnostics
