In a significant and somber development for the American aerospace industry, NASA has officially classified the 2024 Boeing Starliner Crew Flight Test (CFT) as a “Type A” mishap. This designation is the space agency’s most severe category for failure, placing the mission’s technical and management breakdowns on a level of scrutiny typically reserved for catastrophic losses.
The reclassification comes following a scathing 312-page internal report that details a cascade of engineering flaws, leadership lapses, and a “fragile” safety culture that nearly resulted in the loss of the spacecraft—or worse.
The Anatomy of a Type A Mishap
According to NASA Procedural Requirements, a Type A mishap is defined by an incident that results in more than $2 million in property damage, the total loss of a vehicle, or a fatality. While astronauts Suni Williams and Butch Wilmore were ultimately returned safely to Earth, the Starliner mission met the criteria due to the “loss of controlled flight” during its approach to the International Space Station (ISS) and the subsequent decision to return the capsule uncrewed.
NASA Administrator Jared Isaacman, who has taken a hardline stance on accountability, noted in a press conference that the financial losses alone exceeded the Type A threshold “by a factor of over a hundred.”
Technical Failures: The Propulsion Crisis
The primary driver of the mishap was a series of critical failures within the Starliner’s propulsion system. As the spacecraft approached the ISS in June 2024, five of its 28 reaction control system (RCS) thrusters abruptly failed. This led to a temporary loss of “six-degree-of-freedom” control, a terrifying prospect when docking a multi-ton vehicle with a $150 billion space station.
The NASA investigation report identified several “proximate causes” for these failures:

Teflon Poppet Extrusion: High heat caused Teflon seals within the thruster valves to bulge, restricting the flow of oxidizer.

Helium Leaks: Seven of the eight helium manifolds in the service module developed leaks. Investigators attributed this to material incompatibility between the seals and the oxidizer.

Design Flaws: Perhaps most shockingly, the report revealed that the propulsion system lacked the required “two-fault tolerance” for deorbit burns—a design deficiency that had existed since the program’s early development but went uncorrected.
A Culture of “Yelling and Mistrust”
Beyond the hardware, the report paints a disturbing picture of the professional environment behind the scenes. Interviews with NASA and Boeing staff described “defensive, unhealthy, and contentious” meetings. One official characterized it as “the ugliest environment I’ve ever been in.”
There was a clear breakdown in communication between NASA’s Commercial Crew Program and the ISS teams. The report suggests that NASA leadership was “overly risk-tolerant,” driven by a desperate desire to have a second provider to compete with SpaceX. This resulted in a “fragile partnership dynamic” where NASA officials were hesitant to challenge Boeing’s technical interpretations for fear the company might abandon the program entirely.
The Human Cost: Stranded in Orbit
The technical uncertainty forced NASA to make the unprecedented decision to return Starliner to Earth empty in September 2024. Consequently, Suni Williams and Butch Wilmore, who launched for an eight-day mission, were left on the ISS for 286 days. They eventually returned in March 2025 aboard a SpaceX Dragon capsule—the very competitor Boeing was meant to supplement.
Financial and Future Implications
The fallout for Boeing has been devastating. The company has recorded over $2 billion in total losses on the Starliner program to date, including a $523 million charge in 2024 alone, according to its SEC filings.
While NASA maintains it still wants a second crew vehicle, the path forward for Starliner is fraught with obstacles. The agency has issued 61 formal recommendations that Boeing must implement before the spacecraft is allowed to carry humans again. Current plans suggest an uncrewed resupply mission could occur in late 2026, but only if the “true technical root cause” of the thruster failures is fully resolved.
For now, the Starliner saga serves as a sobering reminder of the unforgiving nature of spaceflight and the dangers of allowing schedule pressure to override engineering rigor. As Isaacman put it, NASA’s new policy is simple: “Stuff has to work.”
